How can I obtain a direct debit form?

Simply download the FMG Direct Debit Form.pdf(51.43 KB) (you will need Acrobat Reader) and complete the following details:

Client ID
Pak ID
Circle the annual or monthly payment option.
Your bank account name
Your bank account number, bank, branch, town and city
Your signature and the date

Then once completed send to us at no cost:

If you do not have Acrobat Reader, you can download it here.

Otherwise you can call us on 0800 366 466 and speak with one of our consultants.  Have your bank account details available at the time of the call and we can load the direct debit over the phone. We’ll then send you out the direct debit form to sign and return. 

Alternatively, if you do not wish to give those details over the phone, we can send out a direct debit form for you to complete and return with your bank account name, details and signature.

Please note that annual direct debit payments will be deducted on the 30th day of the month prior to your renewal date.
