Attach a certified copy of your academic results to date, labelled "FirstNameLastName_results"

Attach a copy of the picture page of your New Zealand Passport. If you do not have a New Zealand passport please attach a certified copy of your New Zealand birth certificate or New Zealand Residency Visa, labelled "FirstNameLastName_ID"
